Documentation about Lino’s permissions system

I worked a bit in the documentation about Lino’s permissions system: Introduction to permissions.

And as usual, this caused a few code optimizations, e.g.

• moved the default roles.py file from lino.projects.std.roles to lino.modlib.users.roles, lino.modlib.office.roles and lino.modlib.accounting.roles.

• I removed the authenticated attribute of UserProfiles
